The first step is to join our growing family of planners looking to simplify their discharge referral process. Currently, you will need an invitation code to join, but the app will always be free to use! Learn More

Add a patient’s insurance, zip, and discharge needs.
The Service Providers that have partnered with REFER.HEALTH have done the heavy-lifting for you! Each provider has entered their full list of services and their service area. Our web app matches these to your requirements.

Preview the matching care profiles.
Our app will automatically generate a list of matching service providers in the patient’s local area.

Approve the matches to give to the patient.
You’re in control! Don’t like some of the matches, exclude them from the list.

Create the Discharge Pamphlet for the patient.
This is the best part. You can print the pamphlet or email a link to the patient or their family. Each service provider on the list will have a QR code they can scan to get additional details for that provider.
